The culture at that facility appears to be quite toxic, the lack of care among some of the so-called carers was appalling.<\/p>\n<p>\u201cThere are systemic problems that have not been resolved.\u201d<\/p>\n<\/blockquote>\n<p>A damning report by the chief psychiatrist into Oaken uncovered \u201cpractices transported from mid to late 20th Century Glenside Hospital\u201d.<\/p>\n<p>\u201cI welcome the chief psychiatrist\u2019s report,\u201d Mr Xenophon said, \u201cbut there needs to be an independent report.\u201d<\/p>\n<p>\u201cWhy did the commonwealth actually give the tick of approval to this facility?
